Note: All payments must be made in the form of a certified check, money order or through Paypal.

Payment must be received no later than June 6th.
No vendors will be admitted on site unless payment has been received.
All monies are non-refundable.

Tables/Chairs
Each vendor will be provided with a 6 ft. table, linen and 2 chairs.
Note To Food Vendors: All Food Vendors Must Bring Their Own Set Up.

Name Tags
Nametags will be provided to all table representatives.

Please Make Note ...
- There will be no access to electricity on site.
- To all Food Vendors, the city requires a temporary, special event health permit. This is required even if you possess a health permit for day-to-day operations.
- Copies of all permits must be brought to the Registration Table the day of the event.
- Each vendor will be escorted to their assigned location. All set up may be done between the hours of 7:30 AM and 10:00 AM. All tables must be fully set no later than 10:00 AM. Each vendor is responsible for the breakdown of their space
- You must bring a copy of this form with you as proof of registration

Right of Refusal: Failure to adhere to the rules and regulations of the appropriate city and state authorities may result in closure of exhibitor's space.
Should this occur, no refunds will be given.Products that promote or depict sex, violence, crime, alcohol and/or drug messages or profanity of any kind are prohibited.

Release
I/we acknowledge that as a vendor at the Children's Sports & Fitness Expo, I/we assume the entire responsibility and liability for losses, damages, and claims deriving out of injury or damages to vendor displays, equipment and other property upon the premises and shall indemnify and hold harmless Boys & Girls High School and Body Sculpt of New York, Inc., their agents, servants, and employees from any such losses, damages and claims.
